What is the Division of Health Service Regulation (DHSR)? DHSR is a branch of the NC Department of Health and Human Services (DHHS) responsible for making sure that people in facilities like PRTFs, adult care homes, and jails receive care that meets legal requirements and are safe. When someone files a complaint and they investigate, or when they do an inspection, they create a report listing their findings. This report is called a survey.

The surveys are publicly available records. If your child has been sent to a PRTF, or if you are considering sending your child to a PRTF, it can be helpful to look up the surveys for that facility. It is not easy to find these surveys on the DHSR website, so we are posting links to them here, organized by the name of the facility.

Please note: The state usually posts new surveys within 7-15 days. This means the information on the website is not necessarily current, and you should continue to check for new postings.
