What is the Division of Health Service Regulation (DHSR)? DHSR is a branch of the NC Department of Health and Human Services (DHHS) responsible for making sure that people in facilities like PRTFs, adult care homes, and jails receive care that meets legal requirements and are safe. When someone files a complaint and they investigate, or when they do an inspection, they create a report listing their findings. This report is called a survey.

The surveys are publicly available records. If your child has been sent to a PRTF, or if you are considering sending your child to a PRTF, it can be helpful to look up the surveys for that facility. It is not easy to find these surveys on the DHSR website, so we are posting links to them here, organized by the name of the facility.
Please note: The state usually posts new surveys within 7-15 days. This means the information on the website is not necessarily current, and you should continue to check for new postings.
Lists of PRTFs in NC and links to the DHSR surveys for each facility
Click on the name of the facility to view the surveys for that PRTF. Please note PRTFs are owned by companies. That company can have several different facilities that are licensed separately, and are treated as separate PRTFs. When that is the case, DHSR surveys are listed under the name of of each facility. These facilities may be called “units” or “cottages.” For your ease and reference, we are listing them here under the name of the Company. The number following the name refers to the number of beds in that facility.
