Brain Injury Survivors, Caregivers and Providers Experiences Across North Carolina
Brain Injuries can be traumatic or non-traumatic: a Traumatic Brain Injury (TBI) is caused by a bump, jolt, or blow to the head as a result of a fall, motor vehicle accident, assault, blast, among other causes. A Non-Traumatic Brain Injury can be the result of an illness, oxygen deprivation, metabolic disorder, aneurysm, cardiac arrest, near drowning experience, substance misuse, among other causes.
Disability Rights North Carolina is a legal advocacy agency that fights for the rights of people with disabilities in North Carolina. This survey will receive feedback from three different perspectives of the brain injury population: survivors, caregivers and community based providers.
A provider is defined as a professional that works directly or indirectly with individuals that have a diagnosed or suspected brain injury. The following are examples of providers, but are not limited to: mental health clinician, healthcare worker, social worker, support group facilitator, advocate (Intimate Partner Violence, Court Coordinator, Housing or Substance Use), teacher, attorney, researcher, first responder, correctional officer or other service worker.
 
GOAL: This survey was created to understand what knowledge gaps impact providers who serve diagnosed or suspected individuals with brain injury. The survey will also ask for insight from survivors and caregivers regarding their providers expertise on brain injury. This is an anonymous survey intended for Brain Injury providers, survivors or their caregivers.
We will use the information submitted towards creating advocacy campaigns that advance dialogue about brain injury screenings and supportive services. The results of this survey will also be used to develop recommendations regarding continuing education for providers to better equip them to serve the brain injury population. The survey results will be published via Disability Rights North Carolina website and shared with the general public.
INSTRUCTIONS: 
Please complete the survey by answering questions about your experience as a provider, survivor or caregiver.
This survey takes approximately 15-20 minutes to complete. If you are completing this survey as a caregiver, family member or provider, please answer from the individuals experience.
We will not be collecting any personal identifiable information.
WARNING:

Distressing or traumatic experiences. This survey asks about past and current experiences shared by many in the Brain Injury community that can be distressing to think about or trigger trauma-related symptoms. Your well-being is more important than any survey, and we encourage you to prioritize your boundaries and self-care.
