Includes questions from the Ohio State University Traumatic Brain Injury Identification Method (OSU TBI-ID) and Symptom Inventory from Mindsource in Colorado

Can be done online through their online portal.

What happens after screening?

Results are then sent to their Resource Facilitator and the RF then sends the individual educational materials.
