Seclusion and Restraint Sample Letter

I am the parent of . I have been informed that school staff recently used restraint on my child. I am very concerned about this restraint, especially because . I am also concerned that ’s Behavior Intervention Plan was not implemented during this incident. Therefore,  I am writing to request that conduct an investigation into the use of restraint on on , and that a copy of the investigation be provided to me.
